You can open this in MS Paint and use the Text function to put your kid's name at the top.
I also made my own activity squares to put in the spaces. I just Googled our activities and copy and pasted the images into MS Paint, resized them, then used the "Text" function.
Print as "Portrait" so they are the correct size.
Laminate everything and put velcro on the back. Enjoy!
